Group Therapy Program

This suite of active-based sessions is designed to offer all the benefits of the existing PHN (Public Health Network) exercise program in conjunction with additional clinical support to allow residents to progress on an individual basis

How does the Group Therapy Program work?

Our experienced allied health staff will conduct a 1:1 assessment using valid and reliable outcome measures to determine baseline resident function. It is suitable for ambulatory and non-ambulatory residents with custom designed exercise programs performed in small groups utilising TheraBand, ankle weights and hand weights.

Wellness Program

Our Wellness Program has been developed in response to the introduction of the new AN-ACC funding model, so we can continue to provide essential one on one allied health care to aged care residents. This program is our holistic approach to health and wellbeing with exercise-based therapy which is individualised to the resident to improve pain management along with mobility, balance and daily function. 

How does the Wellness Program work?

Our experienced allied health professionals will conduct a needs-based assessment using clinical reasoning and triage skills in consultation with the resident and on-site care staff. In collaboration, they will then determine allied health goals and the targeted therapy required to achieve them. Our allied health professionals will then structure the reablement journey to align with our fully managed Wellness Program that involves an adaptable 2 x 30-minute sessions per week. Regular outcome measures and quality of life indicators will be used to assess the resident’s progress

Restorative Services

Our Restorative Services deliver essential evidence-based care to older Australians with higher needs, provided by our team of Allied Health professionals. This program allows for additional time with residents who have acute injuries, returned from the hospital, decreased mobility and functional decline, and recent falls, and are designed to offer on-call Allied Health expertise for various acute, sub-acute, and chronic conditions requiring intensive or extended treatment periods.

How does Restorative Services work?

An Allied Health Professional will attend your site post-referral receipt where a comprehensive review and treatment program determined by assessment will be completed. One of our clinicians will be making visits frequently as needed and as agreed by the facility to support the resident’s needs. Progress notes, outcome measures, and handovers are completed each session.

User Pay Services

Do your residents require additional Allied Health support? We Can Help. Our User Pay Services provide one-on-one resident care outside of the traditional funding streams where a resident or resident family would like to engage our services directly. It provides an extra layer of focused one-on-one care which the resident might feel is beneficial for their wellness journey at the time with this short-term ongoing program incorporating features of our wellness and restorative programs regularly assessed.

How does User Pay Services work?

A resident, resident family or facility representative can engage our services directly via our referral system. Once a referral is received, a member of our support team will reach out to discuss the requested service and a member of our team will attend the aged care facility to organise an agreed schedule. Each tailored session delivered will be accompanied by an on-site progress note and outcomes measures recorded as appropriate. For ease of compliance requirements and safety protocols, the Residential Aged Care Facility will be maintained as the conduit between parties for payments.

  • WHS Manual Handling Training

    Our WHS Manual Handling Training program empowers nurses and carers to become manual handling champions to reinforce a positive manual handling culture and reduce workplace injuries. Vivir Healthcare has an extensive network of Physiotherapists and Occupational Therapists who are equipped with the latest ergonomic advice and codes of conduct that influence manual handling in each state.
